the best place to start is to read this review article, it covers most
of your questions:
Human auditory steady-state responses.
<http://www.ncbi.nlm.nih.gov/pubmed/12790346> Picton TW, John MS,
Dimitrijevic A, Purcell D.Int J Audiol. 2003 Jun;42(4):177-219.
the choice of stimuli really depends on what your research question is
.. brainstem vs cortical .... modulated noise will give you the biggest
response (but obviously not frequency specific)
ER3s and ER2s are good, as is through loud speakers
cheers
